Documentation helps Accessibility Services understand your needs and determine appropriate accommodations. You can submit documentation in one of the ways below.

How to Submit Documentation

Please review the documentation guidelines before submitting your materials. This will help make sure you send the correct information.

Option 1: Documentation from a Qualified Professional

A qualified professional may complete the required form and submit it to Accessibility Services. Be sure to review the guidelines below for more information about who qualifies.

  1. Complete the Accessibility Documentation Form.
  2. Submit the form using the form’s Submit button, email it to accessibility@riverland.edu, or fax it to 507-433-0515.

Option 2: Submit Your High School Evaluation Report

If you had an IEP in high school, you may submit your most recent Evaluation Report or ask your high school to send it to Riverland on your behalf.
